Transplant Basics:
According to the United Network for Organ Sharing, transplants have increased by 20% over the last five years. In 2016, more than 33,000 transplants were performed in the U.S. Our program is designed to provide members with education, support, and advocacy. It is also designed to help you coordinate services throughout the transplant process. Patients and their families require assistance to understand organ allocation, living donor options, living with devices, multi-center waiting list placement, and financing options for a transplant.

Our Transplant Management Program
A registered case manager with experience in transplant management will contact you to help obtain and coordinate necessary health care services, while assisting to maximize health care benefits.  Your case manager will act as a resource regarding transplant information, cost issues, community resources, and care options by providing:

• Pre-authorization and medical necessity review for the transplant procedure
• Member education
• Care coordination and interaction with home-health clinical staff to assure a smooth transition from hospital to home
• Ongoing support for the member and their family
• Collaboration with specialty pharmacy services

Who is eligible for the program?
Not all candidates for a transplant proceed to a successful transplant. Precertification for transplant services is required, which affords the opportunity to identify members prior to transplant.
