Disease Management

Our disease management program focuses on helping members with their chronic conditions through early intervention and awareness of appropriate treatment and lifestyle changes. 

Disease management empowers members with condition-specific information so they can make informed decisions to improve their self-care. Our disease management programs are confidential, completely voluntary, and provided at no additional cost or obligation to members. Our team is here to motivate the individual with a greater understanding of ways to manage their chronic condition — giving them a voice in the delivery of their care. 

Our disease management program focuses on the following conditions: 

  • Asthma
  • Chronic obstructive pulmonary disease (COPD)
  • Congestive heart failure (CHF)
  • Coronary artery disease (CAD)
  • Depression
  • Diabetes
  • Obstructive sleep apnea
  • Spine conditions/back pain
  • Hip and Knee
  • Hypertension/hyperlipidemia
  • Mental Health
  • Substance Use Disorder

With help from one of our disease management professionals, members will be educated about their specific health conditions and learn how to take a more active role in their health care. Members will receive education, tools, and one-on-one support to help with: 

  • Reducing complications
  • Understanding of the members' chronic condition(s)
  • Strengthening the relationship of the member with their health care team
  • Feeling empowered to make informed health decisions

Our disease management team consists of:

  • Registered nurses
  • Heart failure specialists
  • Licensed practical nurses (LPN)
  • Certified diabetic educators
  • Social workers
  • Respiratory therapists
  • Certified asthma educators
  • Outreach associates
  • Certified holistic health educators

Who is eligible?

All Highmark Blue Shield of Northeastern New York members with the above condition(s) diagnoses are eligible for these services. A comprehensive assessment of the member is completed by a registered nurse. Based on the need, they may refer the member to a registered dietitian, social worker, respiratory therapist, behavioral health specialist, etc. Members are identified through continuous case finding efforts, welcome calls, claims, and referrals.
