The Asthma management Program encourages members to take charge of their asthma by using their health care team to develop personalized management tools.
Patient self-care education starts early in the disease to prevent life changing complications.
Who is eligible and how are members identified for the Program?
- All members with a diagnosis of asthma under the age of 80 are eligible to join the Program.
- Members can be identified for eligibility through:
- Out-patient medical claims.
- In-patient hospital claims.
- Pharmacy claims.
- Members can be referred to the Program by their health care providers.
- Members can enroll themselves in the Program.
What services are offered to our members who enroll?
- Personalized educational material regarding asthma
- Free, telephonic health coaching by our Staff of Registered nurses for our higher risk members
- Newsletter articles
- Web links to nationally recognized leaders in Asthma Management
- Web-based Program information and self-management tools
- Personalized medication profile and wellness checklist to promote effective use of "Controller" medications
- Comprehensive prescription formulary
- Pediatric or Adult Peak Flow Meters
- On-line enrollment and “Opt-out options
