Cardiac Management Program
The Cardiac Management Program was developed to address the growing concerns of all aspects of cardiac care: prevention, special needs after a cardiac event and chronic needs of our members with congestive heart failure.
Prevention of cardiac disease starts early and in many cases before there is a diagnosis of heart disease. We focus our efforts on members with diabetes (high blood sugar), high blood pressure (hypertension) and high cholesterol (hyperlipidemia). We know that by effectively managing these 3 conditions can prevent the serious and often life threatening effects of heart disease.
Proper management of your health after a heart attack, stent placement and open heart surgery can be challenging. We work with you and your health care team to promote healing and make the most of your recovery.
For our members with congestive heart failure, we focus our attention on improving your quality of life. By providing education regarding diet, weight management and proper use of medication, we work with you and your doctor to prevent unwanted hospital visits that prevent you from getting the most out of life.
Who is eligible and how are members identified for the Program?
- All members with diagnosis of a heart attack and certain surgical procedures are eligible to join the Program.
- Members are identified for eligibility through:
- In-patient hospital claims
- Out-patient medical claims
- Specialized predictive modeling software
- Members can be referred to the Program by their health care practitioner
What services are offered to our members who enroll?
- Personalized Educational materials regarding heart disease and illnesses related to heart disease
- Newsletter articles
- Web links to nationally recognized leaders in Heart Disease Management
- Free, telephonic health coaching by one staff of Registered Nurse for our higher risk members
- Web based program information and self-management tools
- Personalized wellness checklist twice a year that promote effective use of medications and supports your healthcare practitioner’s plan of treatment
- Comprehensive prescription formulary
- On-line enrollment and opt-out options
- Partnerships with leaders in heart disease behavioral modification
