Get Started - Preventive Health

Preventive Health Program

Our Preventive Health Program works with you and your healthcare practitioner to assure you receive the most current recommended health screenings, vaccinations and health education available.

We are committed to addressing the preventive health needs of newborns, children, adolescents and adults. We work closely with local physician advisory groups to deliver a message consistent with the needs of our community.

Who is eligible and how are members identified for the Program?

  • All members with preventive health benefits
  • members are identified by
    1. Employer groups
    2. Preventive Health Benefits
    3. Out-patient medical claims

What services are offered to our members who enroll?

A Guide to Parenting. A comprehensive look at the needs of newborns and young children
  • "Milestone Cards" - parents of children ages 1-2 and 4 years old and adolescents, ages 11 and 17 years receive notification of the healthcare needs of children in those ages
  • Personalized wellness checklist that encourages age appropriate health screening
