Case Management Services
At BlueShield of Northeastern New York, our Case Management program is dedicated to ensuring that members receive all the necessary health services needed to live a healthy lifestyle. This voluntary program is a collaborative effort to assess, plan, coordinate, monitor and evaluate healthcare options and services that fit your needs. As part of this program, an experienced case manager will work with your physician and other members of your health team to offer the benefits you need to receive proper care. Through the use of service plan benefits and community-based resources, your case manager will provide a diverse, innovative and integrated management of care.
Eligible Members
If you are identified as an existing member with certain catastrophic or chronic care needs, you may be referred by your physician or contacted directly by one of our case managers for enrollment in our program.
Case Management Services
There are several different types of case management. Our service ranges from addressing the general needs of our members to focusing on specialized and disease specific care. Our types of case management include:
- General Case Management
- Transplant Case Management
- End of Life Case Management
- Maternal-Child Management
- Chronic Kidney Disease Case Management
- Rare Conditions Case Management
(i.e. Seizure disorders, Parkinson's disease, Sickle Cell Anemia, etc.)

- A customized care plan that meets your health team goals
- Telephone support from an experienced case manager
- Disease education and monitoring from home
- Assistance of a registered nurse
- Help locating community-based resources
