Get Started Diabetes

Diabetes Management Program

Our Diabetes Management Program was designed to promote compliance to diabetic care requirements and raise awareness to the life threatening effects of poorly controlled diabetes. The outcome is an enhanced quality of life with fewer diabetes related complications.

Member education states early in the disease process to prevent complications.

Who is eligible and how are members identified for the Program?

  • All members under the age of 80 are eligible to join the Program
  • Members are identified to be eligibility through
    1. In-patient hospital claims
    2. Out-patient medical claims
    3. Pharmacy claims
  • Members can be referred to the Program by their health care provider
  • Members can enroll themselves in the Program

What services are offered to our members who enroll?

  • Personalized educational material regarding diabetes
  • Free, telephonic health coaching by one staff of Registered nurses for our higher risk members
  • Newsletter articles
  • Web links to nationally recognized leaders in Diabetes Management
  • Web based program information and self management tools
  • Personalized wellness checklists two times a year to promote compliance with recommended screening tests and treatment goals
  • Comprehensive prescription formulary
